เมื่อวันที่ 27 ก.ย. นพ.โสภณ เอี่ยมศิริถาวร ผู้อำนวยการกองโรคติดต่อทั่วไป กรมควบคุมโรค(คร.) กล่าวว่าสำหรับสถานการณ์ผู้ติดเชื้อไวรัสโคโรนา 2019 ทั่วโลก รายงานวันนี้ มีผู้ติดเชื้อรายใหม่ 294,650 ราย ทำให้มีผู้ติดเชื้อสะสมทั่วโลก 33,053,144 ราย โดยประเทศที่มีการติดเชื้อสะสมสูงสุด 3 อันดับแรก ยังคงเป็น สหรัฐอเมริกา 7,287,561 ราย อินเดีย 5,990,581 ราย และบราซิล 4,718,115 ราย จะเห็นได้ว่าสถานการณ์ทั่วโลกยังคงมีแนวโน้มระบาดต่อเนื่อง<br />
.<br />
สำหรับในแถบเอเชีย ประเทศเมียนมา ซึ่งเป็นประเทศที่มีชายแดนติดกับประเทศไทยทางตะวันตก ยังคงมีผู้ป่วยเพิ่มขึ้นอย่างต่อเนื่อง โดยข้อมูลวันที่ 26 กันยายน 2563 พบผู้ติดเชื้อเพิ่มขึ้นจำนวน 880 ราย ทำให้มีผู้ป่วยสะสมทั้งสิ้น 9,991 ราย ซึ่งรัฐบาลเมียนมาได้ประกาศให้ประชาชนในเมืองอย่างน้อย 11 รัฐและภูมิภาคเข้าสู่มาตรการ &ldquo;อยู่บ้าน&rdquo; หรืองดออกจากบ้านหากไม่มีกิจจำเป็น เพื่อลดอัตราการติดเชื้อโควิด 19 ซึ่งพรมแดนของประเทศไทยที่ติดกับประเทศเมียนมานั้น มีการเฝ้าระวังอย่างเข้มงวด บูรณาการความร่วมมือจากทุกภาคส่วน มีด่านควบคุมโรคระหว่างประเทศและมีเครือข่ายหน่วยงานสาธารณสุขในพื้นที่ อาสาสมัครสาธารณสุขประจำหมู่บ้าน ท้องถิ่น กำนันผู้ใหญ่บ้าน หน่วยงานด้านความมั่นคง ป้องกันแรงงานต่างด้าวลักลอบเข้าเมืองอย่างผิดกฎหมาย หากสงสัยขอให้แจ้งเจ้าหน้าที่ทราบเพื่อดำเนินมาตรการป้องกันการแพร่ระบาดของโรคโควิด 19<br />

&ldquo;นอกจากนี้ ขอความร่วมมือร้านจำหน่ายยาแผนปัจจุบัน หากสังเกตพบจำนวนแรงงานต่างด้าวไม่สบายมาซื้อยาแก้ไข้หวัดรักษาตนเองในจำนวนเพิ่มขึ้นกว่าปกติ ขอให้สอบถามข้อมูลและแจ้งไปยังสำนักงานสาธารณสุขจังหวัดในพื้นที่ เพื่อตรวจสอบและเฝ้าระวังการแพร่เชื้อโควิด 19 ในชุมชน&rdquo; นพ.โสภณ กล่าว<br />

ทั้งนี้ เพื่อป้องกันการแพร่ระบาดของเชื้อโควิด 19 ในประเทศไทย กระทรวงสาธารณสุขขอความร่วมมือให้ประชาชนปฏิบัติตามมาตรการป้องกันตนเอง เน้นย้ำสวมหน้ากากอนามัย/หน้ากากผ้าอย่างถูกวิธี ตลอดเวลาที่อยู่ในที่สาธารณะ เลี่ยงการรวมกลุ่มขนาดใหญ่ สถานที่แออัด ล้างมือบ่อยๆ เว้นระยะห่างระหว่างผู้อื่นเท่าที่ทำได้ และลงทะเบียนเข้าออกสถานที่ที่ใช้บริการผ่าน &ldquo;ไทยชนะ&rdquo; ทุกครั้ง หากป่วยขอให้อยู่บ้านและรักษาตัวให้หายป้องกันการนำเชื้อแพร่สู่ผู้อื่น เพื่อความปลอดภัยของตนเอง คนในครอบครัว ชุมชน สังคม<br />

สำหรับสถานการณ์ผู้ติดเชื้อไวรัสโคโรนา 2019 ในประเทศไทยวันนี้ มีผู้ติดเชื้อรายใหม่ 1 ราย เป็นผู้เดินทางจากต่างประเทศ (สหรัฐอเมริกา 1 ราย) เข้ากักตัวในสถานที่รัฐกำหนด มีผู้ป่วยกลับบ้านได้ 5 ราย ทำให้มีผู้ป่วยกลับบ้านสะสม 3,367 ราย หรือคิดเป็นร้อยละ 95.57 ของผู้ป่วยทั้งหมด มีผู้ป่วยที่ยังรักษาอยู่ในโรงพยาบาล 97 ราย หรือร้อยละ 2.75 ของผู้ป่วยทั้งหมด ไม่มีผู้เสียชีวิตเพิ่ม รวมผู้เสียชีวิตสะสม 59 ราย ผู้ป่วยสะสมทั้งสิ้น 3,523 ราย<br />

ส่วนผู้ติดเชื้อรายใหม่วันนี้ เป็นเพศชาย สัญชาติไทย อายุ 24 ปี อาชีพนักศึกษา เดินทางมาจากประเทศสหรัฐอเมริกา ถึงประเทศไทย เมื่อวันที่ 22 กันยายน 2563 เข้ากักตัวในสถานที่รัฐจัดให้ (State Quarantine) จ.ชลบุรี ตรวจครั้งแรกวันที่ 25 กันยายน 2563 (วันที่ 3 ของการกักตัว) ผลพบเชื้อ ไม่มีอาการ เข้ารับการรักษาที่โรงพยาบาลในจังหวัดชลบุรี</p>

<p><strong>ข้อเคลื่อน (Dislocation)</strong>&nbsp;&nbsp;คือ การหลุดของข้อออกจากตำแหน่งปกติ ทำให้เยื่อหุ้มข้อนั้นมีการฉีกขาดหรือมีการยึดของกล้ามเนื้อ การที่ข้อเคลื่อนอาจมีอันตรายต่อเส้นประสาท และหลอดเลือดใกล้เคียง</p>

<p><strong>กระดูกหัก (Fracture)</strong>&nbsp;คือ ภาวะที่มีการแตกหรือหักของกระดูก บริเวณที่หักอาจจะแตกเป็นรอย จะเคลื่อนออกจากกันหรือไม่ก็ได้ บางครั้งแตกเป็นหลายชิ้น ขึ้นอยู่กับลักษณะการบาดเจ็บและความรุนแรงของอุบัติเหตุกระดูกหักอาจเป็นสาเหตุทำให้เกิดความพิการ หรือในบางรายอาจทำให้เสียชีวิตได้</p>

<p><span style="font-size:12px;"><strong>กระดูกหักแบ่งออกเป็น 2 ชนิด คือ&hellip;</strong></span><br />
<strong>กระดูกหักแบบปิด</strong> เป็นเพียงการแตกของกระดูกที่ไม่มีการฉีกขาดเหนือบริเวณผิวหนัง กระดูกหักชนิดนี้จะไม่มีบาดแผลภายนอก<br />
<strong>กระดูกหักแบบเปิด</strong> เป็นกระดูกหักที่มีบาดแผลเปิดจากผิวหนังผ่านเนื้อเยื่อเข้าไปยังตำแหน่งที่หัก โดยอาจมองเห็นปลายกระดูกโผล่ออกมาทางบาดแผล กระดูกหักชนิดนี้จึงมีความเสี่ยงต่อการติดเชื้อได้ง่าย</p>

<p><strong>อาการกระดูกหักและข้อเคลื่อน</strong></p>

<ol>
	<li>มีความเจ็บปวด บวม บริเวณที่มีกระดูกหัก หรือข้อเคลื่อน</li>
	<li>สีของผิวหนัง รูปร่างของอวัยวะนั้นๆเปลี่ยนแปลงไปจากรูปร่างปกติ</li>
	<li>ไม่สามารถเคลื่อนไหวบริเวณนั้นได้ หรือเคลื่อนไหวแล้วจะเจ็บปวดมาก</li>
	<li>มีการหดสั้นของอวัยวะ เช่น แขน&nbsp;</li>
	<li>อาจคลำพบปลายหรือหัวกระดูกที่เคลื่อนหรือหักออกมาออกมา</li>
</ol>

<p><strong>การปฐมพยาบาลผู้ป่วยกระดูกหักและข้อเคลื่อน</strong></p>

<ol>
	<li>อย่าพยายามดึงข้อ หรือจัดกระดูกให้เข้าที่ด้วยตนเอง</li>
	<li>หากจำเป็นต้องถอดเสื้อผ้าออก ควรใช้วิธีตัดตามตะเข็บ</li>
	<li>ใช้วัสดุที่หาได้หนุนหรือประคองข้อให้บวิเวณที่บาดเจ็บนั้นอยู่นิ่ง ๆและอยู่ในท่าที่สบาย ด้วยการใช้ผ้าพยุง หรือดามไว้ด้วยเฝือกชั่วคราวให้อยู่ในท่าพัก</li>
	<li>ประคบบริเวณที่บาดเจ็บด้วยความเย็น เช่นผ้าหรือถุงใส่น้ำแข็ง</li>
	<li>การเคลื่อนย้ายผู้บาดเจ็บ เพื่อเคลื่อนย้ายออกจากสถานที่มีอันตรายไปสู่ที่ปลอดภัยหรือโรงพยาบาล การเคลื่อนย้ายอย่างถูกวิธี จะช่วยลดความพิการและอันตรายต่างๆ ที่จะเกิดขึ้นได้</li>
	<li>รีบนำส่งโรงพยาบาล</li>
</ol>

<p><strong>หลักการเข้าเฝือกชั่วคราว</strong></p>

<ol>
	<li>วัสดุที่ใช้ดามต้องยาวกว่าอวัยวะส่วนที่หัก โดยเฉพาะจะต้องยาวพอที่จะบังคับข้อต่อที่อยู่เหนือและใต้ บริเวณที่สงสัยว่ากระดูกหัก เช่น ขาท่อนล่างหัก ข้อเข่าและข้อเท้าจะต้องถูกบังคับไว้ด้วยเฝือก เป็นต้น</li>
	<li>ไม่วางเฝือกลงบนบริเวณที่กระดูกหักโดยตรง ควรมีสิ่งอื่นรอง เช่น ผ้า หรือ สำลีวางไว้ตลอดแนวเฝือก เพื่อไม่ให้เฝือกกดลงบนบริเวณผิวหนังโดยตรง ซึ่งทำให้เจ็บปวดและเกิดเป็นแผลจากเฝือกกดได้</li>
	<li>มัดเฝือกกับอวัยวะที่หักให้แน่นพอที่จะประคองส่วนที่หักได้ แต่ต้องไม่แน่นเกินไป ถ้ารัดแน่นจนเกินไปจะกดขัดขวางการไหลเวียนของเลือดทำให้เกิดอันตรายได้ ต้องระวังอย่าให้ปมเชือกกดแผล และคอยตรวจบริเวณที่หักเป็นระยะๆเพราะอาจจะมีการบวม ซึ่งจะต้องคลายเชือกที่ผูกให้แน่นน้อยลง</li>
	<li>บริเวณที่เข้าเฝือกจะต้องจัดให้อยู่ในท่าที่สุขสบายที่สุด อย่าจัดกระดูกให้เข้ารูปเดิม ไม่ว่ากระดูกที่หักจะโค้ง โก่ง หรือ คด ก็ควรเข้าเฝือกในท่าที่เป็นอยู่</li>
</ol>

<p><strong>การเข้าเฝือกชั่วคราว&hellip;ไม่ยากอย่างที่คิด</strong><br />
<strong>1. เลือกวัสดุที่ใช้ให้เหมาะสม</strong> ควรเลือกใช้วัสดุสำหรับทำเฝือกชั่วคราว เช่น ไม้กระดาน, กิ่งไม้ หรือกระดาษแข็ง โดยให้มีความยาวกว่าอวัยวะส่วนที่หัก และยังสามารถบังคับข้อต่อที่อยู่เหนือและใต้ส่วนที่หักได้<br />
<strong>2. ระวังเรื่องการรับน้ำหนัก</strong> ไม่วางส่วนที่หักสัมผัสกับเฝือกโดยตรง ควรมีสิ่งที่รองรับส่วนที่บาดเจ็บ โดยอาจเป็นเสื้อผ้า ผ้าห่ม หรือสำลีวางเรียงบนวัสดุแข็ง เป็นต้น<br />
<strong>3. มัดให้ดี แต่อย่าแน่นเกินไป</strong> มัดเฝือกกับอวัยวะที่หักให้แน่นพอที่จะประคองส่วนที่หักได้ อย่าให้แน่นจนเกินไปเพราะจะส่งผลต่อการไหลเวียนของเลือด หากสังเกตพบว่าบริเวณที่หักมีอาการบวม ให้ค่อยๆคลายเชือกที่ผูกให้หลวมมากขึ้น<br />
<strong>4. จัดวางตำแหน่งให้เหมาะสม</strong> ในกรณีกระดูกแขนหัก ควรให้มือยกสูงกว่าข้อศอกเสมอ โดยอาจใช้ผ้ามาผูกสำหรับแขวนคอ ควรเป็นผ้าชิ้นยาวๆ เพื่อให้ระดับของแขนอยู่ในตำแหน่งที่เหมาะสม<br />
&nbsp;</p>

<p><strong>หากไม่แน่ใจว่ากระดูกหักหรือไม่? ให้สังเกตอาการเจ็บปวด บวม สีของผิวหนัง รูปร่างของอวัยวะว่ามีการเปลี่ยนแปลงไปจากปกติหรือไม่ ทางที่ดีที่สุด ควรโทรติดต่อสถานพยาบาลใกล้เคียงและไม่ควรเคลื่อนย้ายเอง เพราะอาจทำให้อาการกระดูกหักรุนแรงมากขึ้น</strong></p>

<p><strong>ข้อเคลื่อน (Dislocation)</strong>&nbsp;&nbsp;คือ การหลุดของข้อออกจากตำแหน่งปกติ ทำให้เยื่อหุ้มข้อนั้นมีการฉีกขาดหรือมีการยึดของกล้ามเนื้อ การที่ข้อเคลื่อนอาจมีอันตรายต่อเส้นประสาท และหลอดเลือดใกล้เคียง</p>

<p><strong>กระดูกหัก (Fracture)</strong>&nbsp;คือ ภาวะที่มีการแตกหรือหักของกระดูก บริเวณที่หักอาจจะแตกเป็นรอย จะเคลื่อนออกจากกันหรือไม่ก็ได้ บางครั้งแตกเป็นหลายชิ้น ขึ้นอยู่กับลักษณะการบาดเจ็บและความรุนแรงของอุบัติเหตุกระดูกหักอาจเป็นสาเหตุทำให้เกิดความพิการ หรือในบางรายอาจทำให้เสียชีวิตได้</p>

<p><span style="font-size:12px;"><strong>กระดูกหักแบ่งออกเป็น 2 ชนิด คือ&hellip;</strong></span><br />
<strong>กระดูกหักแบบปิด</strong> เป็นเพียงการแตกของกระดูกที่ไม่มีการฉีกขาดเหนือบริเวณผิวหนัง กระดูกหักชนิดนี้จะไม่มีบาดแผลภายนอก<br />
<strong>กระดูกหักแบบเปิด</strong> เป็นกระดูกหักที่มีบาดแผลเปิดจากผิวหนังผ่านเนื้อเยื่อเข้าไปยังตำแหน่งที่หัก โดยอาจมองเห็นปลายกระดูกโผล่ออกมาทางบาดแผล กระดูกหักชนิดนี้จึงมีความเสี่ยงต่อการติดเชื้อได้ง่าย</p>

<p><strong>อาการกระดูกหักและข้อเคลื่อน</strong></p>

<ol>
	<li>มีความเจ็บปวด บวม บริเวณที่มีกระดูกหัก หรือข้อเคลื่อน</li>
	<li>สีของผิวหนัง รูปร่างของอวัยวะนั้นๆเปลี่ยนแปลงไปจากรูปร่างปกติ</li>
	<li>ไม่สามารถเคลื่อนไหวบริเวณนั้นได้ หรือเคลื่อนไหวแล้วจะเจ็บปวดมาก</li>
	<li>มีการหดสั้นของอวัยวะ เช่น แขน&nbsp;</li>
	<li>อาจคลำพบปลายหรือหัวกระดูกที่เคลื่อนหรือหักออกมาออกมา</li>
</ol>

<p><strong>การปฐมพยาบาลผู้ป่วยกระดูกหักและข้อเคลื่อน</strong></p>

<ol>
	<li>อย่าพยายามดึงข้อ หรือจัดกระดูกให้เข้าที่ด้วยตนเอง</li>
	<li>หากจำเป็นต้องถอดเสื้อผ้าออก ควรใช้วิธีตัดตามตะเข็บ</li>
	<li>ใช้วัสดุที่หาได้หนุนหรือประคองข้อให้บวิเวณที่บาดเจ็บนั้นอยู่นิ่ง ๆและอยู่ในท่าที่สบาย ด้วยการใช้ผ้าพยุง หรือดามไว้ด้วยเฝือกชั่วคราวให้อยู่ในท่าพัก</li>
	<li>ประคบบริเวณที่บาดเจ็บด้วยความเย็น เช่นผ้าหรือถุงใส่น้ำแข็ง</li>
	<li>การเคลื่อนย้ายผู้บาดเจ็บ เพื่อเคลื่อนย้ายออกจากสถานที่มีอันตรายไปสู่ที่ปลอดภัยหรือโรงพยาบาล การเคลื่อนย้ายอย่างถูกวิธี จะช่วยลดความพิการและอันตรายต่างๆ ที่จะเกิดขึ้นได้</li>
	<li>รีบนำส่งโรงพยาบาล</li>
</ol>

<p><strong>หลักการเข้าเฝือกชั่วคราว</strong></p>

<ol>
	<li>วัสดุที่ใช้ดามต้องยาวกว่าอวัยวะส่วนที่หัก โดยเฉพาะจะต้องยาวพอที่จะบังคับข้อต่อที่อยู่เหนือและใต้ บริเวณที่สงสัยว่ากระดูกหัก เช่น ขาท่อนล่างหัก ข้อเข่าและข้อเท้าจะต้องถูกบังคับไว้ด้วยเฝือก เป็นต้น</li>
	<li>ไม่วางเฝือกลงบนบริเวณที่กระดูกหักโดยตรง ควรมีสิ่งอื่นรอง เช่น ผ้า หรือ สำลีวางไว้ตลอดแนวเฝือก เพื่อไม่ให้เฝือกกดลงบนบริเวณผิวหนังโดยตรง ซึ่งทำให้เจ็บปวดและเกิดเป็นแผลจากเฝือกกดได้</li>
	<li>มัดเฝือกกับอวัยวะที่หักให้แน่นพอที่จะประคองส่วนที่หักได้ แต่ต้องไม่แน่นเกินไป ถ้ารัดแน่นจนเกินไปจะกดขัดขวางการไหลเวียนของเลือดทำให้เกิดอันตรายได้ ต้องระวังอย่าให้ปมเชือกกดแผล และคอยตรวจบริเวณที่หักเป็นระยะๆเพราะอาจจะมีการบวม ซึ่งจะต้องคลายเชือกที่ผูกให้แน่นน้อยลง</li>
	<li>บริเวณที่เข้าเฝือกจะต้องจัดให้อยู่ในท่าที่สุขสบายที่สุด อย่าจัดกระดูกให้เข้ารูปเดิม ไม่ว่ากระดูกที่หักจะโค้ง โก่ง หรือ คด ก็ควรเข้าเฝือกในท่าที่เป็นอยู่</li>
</ol>

<p><strong>การเข้าเฝือกชั่วคราว&hellip;ไม่ยากอย่างที่คิด</strong><br />
<strong>1. เลือกวัสดุที่ใช้ให้เหมาะสม</strong> ควรเลือกใช้วัสดุสำหรับทำเฝือกชั่วคราว เช่น ไม้กระดาน, กิ่งไม้ หรือกระดาษแข็ง โดยให้มีความยาวกว่าอวัยวะส่วนที่หัก และยังสามารถบังคับข้อต่อที่อยู่เหนือและใต้ส่วนที่หักได้<br />
<strong>2. ระวังเรื่องการรับน้ำหนัก</strong> ไม่วางส่วนที่หักสัมผัสกับเฝือกโดยตรง ควรมีสิ่งที่รองรับส่วนที่บาดเจ็บ โดยอาจเป็นเสื้อผ้า ผ้าห่ม หรือสำลีวางเรียงบนวัสดุแข็ง เป็นต้น<br />
<strong>3. มัดให้ดี แต่อย่าแน่นเกินไป</strong> มัดเฝือกกับอวัยวะที่หักให้แน่นพอที่จะประคองส่วนที่หักได้ อย่าให้แน่นจนเกินไปเพราะจะส่งผลต่อการไหลเวียนของเลือด หากสังเกตพบว่าบริเวณที่หักมีอาการบวม ให้ค่อยๆคลายเชือกที่ผูกให้หลวมมากขึ้น<br />
<strong>4. จัดวางตำแหน่งให้เหมาะสม</strong> ในกรณีกระดูกแขนหัก ควรให้มือยกสูงกว่าข้อศอกเสมอ โดยอาจใช้ผ้ามาผูกสำหรับแขวนคอ ควรเป็นผ้าชิ้นยาวๆ เพื่อให้ระดับของแขนอยู่ในตำแหน่งที่เหมาะสม<br />
&nbsp;</p>

<p><strong>หากไม่แน่ใจว่ากระดูกหักหรือไม่? ให้สังเกตอาการเจ็บปวด บวม สีของผิวหนัง รูปร่างของอวัยวะว่ามีการเปลี่ยนแปลงไปจากปกติหรือไม่ ทางที่ดีที่สุด ควรโทรติดต่อสถานพยาบาลใกล้เคียงและไม่ควรเคลื่อนย้ายเอง เพราะอาจทำให้อาการกระดูกหักรุนแรงมากขึ้น</strong></p>

<p><strong>จำแนกตามพระราชบัญญัติยา&nbsp; 5&nbsp; กลุ่ม</strong></p>

<p><strong>1. ยาสามัญประจำบ้าน</strong>&nbsp; คือ ยาที่กระทรวงสาธารณสุขคัดเลือกไว้ให้ประชาชนสามารถเลือกใช้ได้เอง&nbsp; หาซื้อได้โดยทั่วไป จะสังเกตได้ว่าจะต้องมีคำว่า <strong>&ldquo;ยาสามัญประจำบ้าน&rdquo;</strong> กำกับไว้บนฉลาก เช่น พาราเซตามอล ยาแก้ไอ เป็นต้น</p>

<p><strong>2. ยาอันตราย</strong> คือ ยาแผนปัจจุบันที่มีอันตรายสูงกว่ายาสามัญประจำบ้าน&nbsp; การใช้ยาประเภทนี้ต้องผ่านการดูแลจากเจ้าหน้าที่สาธารณสุข เช่น แพทย์ เภสัชกร ยากลุ่มนี้จะมีคำว่า<strong>&ldquo;ยาอันตราย&rdquo;</strong> ระบุไว้บนฉลากข้างขวดหรือภาชนะที่บรรจุยา</p>

<p><strong>3. ยาแผนปัจจุบันบรรจุเสร็จที่มิใช่ยาอันตราย</strong> คือ ยาแผนปัจจุบันที่ผลิตขึ้นในรูปแบบต่างๆ ทางเภสัชกรรม มีบรรจุหีบห่อปิดไว้ มีฉลากครบถ้วนและเป็นยาที่กระทรวงสาธารณสุขไม่ได้จัดให้เป็นยาสามัญประจำบ้าน ยาควบคุมพิเศษ ยาที่ออกฤทธิ์ต่อจิตประสาทหรือยาเสพติด&nbsp; เนื่องจากเห็นว่าเป็นที่ยาที่ค่อนข้างมีความปลอดภัย เช่น ยาแก้ไข้หวัดสูตรผสม และยาที่ใช้ภายนอกที่โฆษณาอย่างแพร่หลาย</p>

<p><strong>4. ยาสมุนไพร</strong> คือ ยาที่ได้จากพืช หรือสัตว์ หรือแร่ธาตุที่ไม่ได้นำไปปรุงแต่งใดๆ&nbsp; เช่น ว่านหางจระเข้ ใบมะขามแขก ตับปลา &nbsp;ดีแกลือ เป็นต้น</p>

<p><strong>5. ยาแผนโบราณ</strong> คือ ยาที่ใช้กันมานานในอดีตเป็นส่วนใหญ่และปัจจุบันยังคงมีใช้อยู่บ้าง&nbsp; ในปัจจุบันยาที่ใช้รักษาโรคแผนโบราณจะต้องขึ้นทะเบียนเป็นตำรับยาแผนโบราณอย่างถูกต้อง เช่น ยาเขียวหอม ยาธาตุบรรจบ เป็นต้น</p>

<p><strong>1. ยาสามัญประจำบ้าน</strong>&nbsp; คือ ยาที่กระทรวงสาธารณสุขคัดเลือกไว้ให้ประชาชนสามารถเลือกใช้ได้เอง&nbsp; หาซื้อได้โดยทั่วไป จะสังเกตได้ว่าจะต้องมีคำว่า <strong>&ldquo;ยาสามัญประจำบ้าน&rdquo;</strong> กำกับไว้บนฉลาก เช่น พาราเซตามอล ยาแก้ไอ เป็นต้น</p>

<p><strong>2. ยาอันตราย</strong> คือ ยาแผนปัจจุบันที่มีอันตรายสูงกว่ายาสามัญประจำบ้าน&nbsp; การใช้ยาประเภทนี้ต้องผ่านการดูแลจากเจ้าหน้าที่สาธารณสุข เช่น แพทย์ เภสัชกร ยากลุ่มนี้จะมีคำว่า<strong>&ldquo;ยาอันตราย&rdquo;</strong> ระบุไว้บนฉลากข้างขวดหรือภาชนะที่บรรจุยา</p>

<p><strong>3. ยาแผนปัจจุบันบรรจุเสร็จที่มิใช่ยาอันตราย</strong> คือ ยาแผนปัจจุบันที่ผลิตขึ้นในรูปแบบต่างๆ ทางเภสัชกรรม มีบรรจุหีบห่อปิดไว้ มีฉลากครบถ้วนและเป็นยาที่กระทรวงสาธารณสุขไม่ได้จัดให้เป็นยาสามัญประจำบ้าน ยาควบคุมพิเศษ ยาที่ออกฤทธิ์ต่อจิตประสาทหรือยาเสพติด&nbsp; เนื่องจากเห็นว่าเป็นที่ยาที่ค่อนข้างมีความปลอดภัย เช่น ยาแก้ไข้หวัดสูตรผสม และยาที่ใช้ภายนอกที่โฆษณาอย่างแพร่หลาย</p>

<p>เมื่อมีอาการปวดเมื่อย หลายคนนึกถึง&nbsp;<strong>&ldquo;ยาแก้อักเสบ&rdquo;</strong>&nbsp;หรือแท้จริงแล้วต้องเรียกว่า<strong>&nbsp;&ldquo;ยาลดการปวดอักเสบ&rdquo;</strong>&nbsp;(anti-inflammatory) ถ้าเรียกเต็มยศจริง ๆ คือ&nbsp;<strong>&ldquo;ยาต้านการอักเสบที่ไม่ใช่สเตอรอยด์&rdquo;</strong>&nbsp;(non-steroidal anti-inflammatory drugs; NSAIDs) ซึ่งมีมากมายหลายสิบชนิด ทั้งในรูปยากิน ยาทาภายนอก หรือยาฉีด ราคาก็มีตั้งแต่ไม่กี่บาท จนเกือบทะลุหลักร้อย ซึ่งสรรพคุณไม่ต่างกันมากเลยคือ ยับยั้งการสังเคราะห์สารเคมีบางชนิดที่เกี่ยวข้องกับการอักเสบ ไม่ให้กระบวนการอักเสบลามปามไปมาก<br />
<br />
การใช้ยาเหล่านี้เหมาะสมสำหรับพยาธิสภาพที่มีการ&nbsp;<strong>&ldquo;อักเสบ&rdquo;&nbsp;</strong>อย่างแท้จริง โดยส่วนใหญ่เป็นโรคทางข้อ เช่น เป็นยาเสริมสำหรับการอักเสบที่กำเริบในผู้ป่วยโรคข้ออักเสบรูมาตอยด์ (Rheumatoid arthritis) หรือการอักเสบของกระดูกสันหลัง (spine) แบบเรื้อรัง เช่น Ankylosing Spondylitis<br />

ในกรณีที่พบบ่อยและแทบจะเป็นสาเหตุลำดับต้น ๆ ของปัญหาอาการแทรกซ้อนจากการใช้ยา NSAIDs แบบไม่ถูกวิธีคือ ใช้ยา NSAIDs ในภาวะที่ไม่ได้มีอาการอักเสบอย่างแท้จริง เช่น ปวดเข่าธรรมดา จากภาวะข้อเสื่อม ก็ใช้ NSAIDs หรือปวดเมื่อยจากการทำงานหนัก แบกหามมากเกิน เคล็ด ขัดยอกจากการทำงานผิดท่า แทนที่จะใช้ยานวดแบบเฉพาะที่ หรือนวดไทย ก็เลือกที่จะกินยา NSAIDs (ยุคหนึ่งในอดีตหลายคนอาจจำได้ที่ผู้ใช้แรงงานมีการติดยาแก้ปวดชนิดหนึ่งอย่างมาก ถ้าไม่กินจะไม่สบายตัว ทำงานไม่ได้ เหมือนติดเครื่องดื่มชูกำลัง) ซึ่งได้เคยกล่าวไปในบทความก่อนหน้านี้แล้วว่า NSAIDs ส่งผลกระทบอะไรบ้างต่อ ซึ่งผลกระทบดังกล่าวไม่ได้เกิดขึ้นทันทีทันใด แต่เป็นผลกระทบในระยะยาว ซึ่งกว่าจะรู้ตัวก็สายไปแล้ว<br />
&nbsp;</p>

<p>กรณีที่เราพบกันบ่อยมากคือ กินยาแก้ปวดจนเลือดออกในกระเพาะอาหาร หรือกระเพาะทะลุ อันเนื่องมาจาก NSAIDs ไปลด &ldquo;กลไกป้องกันตัว&rdquo; (defensive mechanisms) ของทางเดินอาหาร เช่น ทำให้เมือกที่เคลือบทางเดินอาหารบางลง มีผลต่อการหลั่งไบคาร์บอเนต ฯลฯ เหล่านี้เมื่อเกิดนาน ๆ ทำให้เสี่ยงต่อการเกิดเลือดออกในทางเดินอาหารในที่สุด<br />
<br />
<strong>ผลกระทบอีกอันที่อาจไม่เห็นทันที</strong>&nbsp;แต่เมื่อเกิดแล้ว อาจไม่ย้อนกลับคืนคือภาวะไตเสื่อมจากยา NSAIDs (NSAIDs-induced nephropathy) เนื่องจาก NSAIDs มีผลลดปริมาณเลือดไปเลี้ยงที่ไต จึงส่งผลต่อการทำหน้าที่ของไต การกิน NSAIDs ไปนาน ค่าการทำงานของไตจะลดลง (สะท้อนด้วยค่า GFR ที่ลดลง แต่ค่า serum creatinine หรือ BUN จะเพิ่มขึ้น) นอกจากนี้ NSAIDs บางชนิดยังเหนี่ยวนำให้เกิดภาวะไตวายเฉียบพลัน (acute renal failure) ได้ด้วย หลายคนที่กินยา NSAIDs มาเป็นเวลานาน ๆ อาจมีปัญหาไตเสื่อม ไตวาย ตามมาภายหลังได้<br />
<br />
ผลกระทบอีกประการที่มักมองข้ามคือเรื่องหัวใจ โดยเฉพาะยาในกลุ่ม COX-2 specific NSAIDs ที่มีอาการข้างเคียงที่สำคัญคือ ทำให้ความดันเลือดสูง และเกิดกล้ามเนื้อหัวใจขาดเลือด (myocardial infarction) ดังนั้น จึงมีคำเตือนพิเศษใน COX-2 specific NSAIDs ให้ระวังการใช้ในกลุ่มผู้ป่วยที่มีปัญหาเรื่องโรคหัวใจ หรือความดันโลหิตสูง บวมน้ำอยู่เดิม<br />
<br />
<strong>ดังนั้น การใช้ยากลุ่ม NSAIDs จึงควรใช้เท่าที่จำเป็น ไม่ใช่ติดต่อกันนานเกิน หรือใช้พร่ำเพรื่อ เพื่อลดความเสี่ยงจากพิษของยา</strong></p>

<p>เมื่อมีอาการปวดเมื่อย หลายคนนึกถึง&nbsp;<strong>&ldquo;ยาแก้อักเสบ&rdquo;</strong>&nbsp;หรือแท้จริงแล้วต้องเรียกว่า<strong>&nbsp;&ldquo;ยาลดการปวดอักเสบ&rdquo;</strong>&nbsp;(anti-inflammatory) ถ้าเรียกเต็มยศจริง ๆ คือ&nbsp;<strong>&ldquo;ยาต้านการอักเสบที่ไม่ใช่สเตอรอยด์&rdquo;</strong>&nbsp;(non-steroidal anti-inflammatory drugs; NSAIDs) ซึ่งมีมากมายหลายสิบชนิด ทั้งในรูปยากิน ยาทาภายนอก หรือยาฉีด ราคาก็มีตั้งแต่ไม่กี่บาท จนเกือบทะลุหลักร้อย ซึ่งสรรพคุณไม่ต่างกันมากเลยคือ ยับยั้งการสังเคราะห์สารเคมีบางชนิดที่เกี่ยวข้องกับการอักเสบ ไม่ให้กระบวนการอักเสบลามปามไปมาก<br />
<br />
การใช้ยาเหล่านี้เหมาะสมสำหรับพยาธิสภาพที่มีการ&nbsp;<strong>&ldquo;อักเสบ&rdquo;&nbsp;</strong>อย่างแท้จริง โดยส่วนใหญ่เป็นโรคทางข้อ เช่น เป็นยาเสริมสำหรับการอักเสบที่กำเริบในผู้ป่วยโรคข้ออักเสบรูมาตอยด์ (Rheumatoid arthritis) หรือการอักเสบของกระดูกสันหลัง (spine) แบบเรื้อรัง เช่น Ankylosing Spondylitis<br />

ในกรณีที่พบบ่อยและแทบจะเป็นสาเหตุลำดับต้น ๆ ของปัญหาอาการแทรกซ้อนจากการใช้ยา NSAIDs แบบไม่ถูกวิธีคือ ใช้ยา NSAIDs ในภาวะที่ไม่ได้มีอาการอักเสบอย่างแท้จริง เช่น ปวดเข่าธรรมดา จากภาวะข้อเสื่อม ก็ใช้ NSAIDs หรือปวดเมื่อยจากการทำงานหนัก แบกหามมากเกิน เคล็ด ขัดยอกจากการทำงานผิดท่า แทนที่จะใช้ยานวดแบบเฉพาะที่ หรือนวดไทย ก็เลือกที่จะกินยา NSAIDs (ยุคหนึ่งในอดีตหลายคนอาจจำได้ที่ผู้ใช้แรงงานมีการติดยาแก้ปวดชนิดหนึ่งอย่างมาก ถ้าไม่กินจะไม่สบายตัว ทำงานไม่ได้ เหมือนติดเครื่องดื่มชูกำลัง) ซึ่งได้เคยกล่าวไปในบทความก่อนหน้านี้แล้วว่า NSAIDs ส่งผลกระทบอะไรบ้างต่อ ซึ่งผลกระทบดังกล่าวไม่ได้เกิดขึ้นทันทีทันใด แต่เป็นผลกระทบในระยะยาว ซึ่งกว่าจะรู้ตัวก็สายไปแล้ว<br />
&nbsp;</p>

<p>กรณีที่เราพบกันบ่อยมากคือ กินยาแก้ปวดจนเลือดออกในกระเพาะอาหาร หรือกระเพาะทะลุ อันเนื่องมาจาก NSAIDs ไปลด &ldquo;กลไกป้องกันตัว&rdquo; (defensive mechanisms) ของทางเดินอาหาร เช่น ทำให้เมือกที่เคลือบทางเดินอาหารบางลง มีผลต่อการหลั่งไบคาร์บอเนต ฯลฯ เหล่านี้เมื่อเกิดนาน ๆ ทำให้เสี่ยงต่อการเกิดเลือดออกในทางเดินอาหารในที่สุด<br />
<br />
<strong>ผลกระทบอีกอันที่อาจไม่เห็นทันที</strong>&nbsp;แต่เมื่อเกิดแล้ว อาจไม่ย้อนกลับคืนคือภาวะไตเสื่อมจากยา NSAIDs (NSAIDs-induced nephropathy) เนื่องจาก NSAIDs มีผลลดปริมาณเลือดไปเลี้ยงที่ไต จึงส่งผลต่อการทำหน้าที่ของไต การกิน NSAIDs ไปนาน ค่าการทำงานของไตจะลดลง (สะท้อนด้วยค่า GFR ที่ลดลง แต่ค่า serum creatinine หรือ BUN จะเพิ่มขึ้น) นอกจากนี้ NSAIDs บางชนิดยังเหนี่ยวนำให้เกิดภาวะไตวายเฉียบพลัน (acute renal failure) ได้ด้วย หลายคนที่กินยา NSAIDs มาเป็นเวลานาน ๆ อาจมีปัญหาไตเสื่อม ไตวาย ตามมาภายหลังได้<br />
<br />
ผลกระทบอีกประการที่มักมองข้ามคือเรื่องหัวใจ โดยเฉพาะยาในกลุ่ม COX-2 specific NSAIDs ที่มีอาการข้างเคียงที่สำคัญคือ ทำให้ความดันเลือดสูง และเกิดกล้ามเนื้อหัวใจขาดเลือด (myocardial infarction) ดังนั้น จึงมีคำเตือนพิเศษใน COX-2 specific NSAIDs ให้ระวังการใช้ในกลุ่มผู้ป่วยที่มีปัญหาเรื่องโรคหัวใจ หรือความดันโลหิตสูง บวมน้ำอยู่เดิม<br />
<br />
<strong>ดังนั้น การใช้ยากลุ่ม NSAIDs จึงควรใช้เท่าที่จำเป็น ไม่ใช่ติดต่อกันนานเกิน หรือใช้พร่ำเพรื่อ เพื่อลดความเสี่ยงจากพิษของยา</strong></p>
